Part of our current restrictions requires that you prepare and sign 3 forms that will be dropped off at the morning registration desk. These include:

• Covid-19 Screening Checklist
  • COVID Appendix C_SASC.pdf
• Waiver for yourself and anyone attending with you
  • StoneRidge_General Waiver.pdf
  • StoneRidge_Parental Consent.pdf
  • StoneRidge_Minor Waiver.pdf
• Self Tech Form
  • Self Tech.pdf

Please bring these forms signed and completed with you in the morning to drop off at contactless registration. We will not have pens available for use on-site given current restrictions in place. If you do not have these forms filled out and signed you will not be able to participate.
