- Completed and signed Annual General Waiver.
- Picture – head and shoulders – in your MSR account profile.
IMPORTANT: This registration must be completed under the name of the Annual Waiver card holder. You can use the 'Register Someone Else' option on the next screen if this card isn't for you.
