• WKA Forms must be filled out PER SEASON. If you have already filled these out please show us your license # day of check in.

(Per minor)  UMC MINOR WAIVER FORM.pdf

• ***Minors (under the age of 18) are required to have parent or guardian accompany them to the event to fill out the Minor Waver Form, OR print out the form online and have your parent or guardian get the document notarized***

 

All forms must be brought to *Check In* when you arrive.
